The contact form collects the details you enter, such as your name, email address, organization, and message. When you send the form, your message is emailed to our team.

The website stores your cookie-notice choice in your browser. We do not currently use advertising or analytics cookies on this public website.
Product demonstration data
Screenshots and the linked demonstration portal show sample residents, staff, and care activity. Do not enter real protected health information into the public demo.
How information is used
Information you send through the contact form is used only to respond to your inquiry and to discuss a demonstration. It is not used for advertising.
Sharing and retention
The website does not store contact-form submissions. Each message is delivered by email through our email service provider, Brevo, and kept in our team mailbox for as long as needed to respond and follow up. Do not include resident health information in the form. AFH Care Portal does not sell personal information collected through this website.
